UCLH Pathology would like to remind GP practices that every Tquest blood request form needs to be unique, and a new request submitted and printed. Practices should not reprint previously used forms as those samples will be read by the automated machines in the lab as testing already been completed. This leads to sample not being processed as it’s sent to storage and discarded after 7days. The lab has automated booking, analytical and storage solutions. While they are highly efficient and far less error prone than manual transcription and handling processes, it is reliant on Tquest forms that have not been used previously.
