We understand that transitioning to a new hospital can be daunting and you might be nervous or anxious. We are here to support you and welcome you to our Adolescent Rheumatology Department.
The transition co-ordinator works as part of the Adolescent Rheumatology Clinic between Great Ormond Street and University College London Hospitals. All patients under the care of the Adolescent Rheumatology Clinic can access the transition coordinator.
We want your transition to be as smooth as possible. You will be given an information pack which includes the information you will need to help you feel confident and comfortable during your treatment.
How can my transition coordinator help me?
Your transition coordinator is here to make your time at University College Hospital as easy as possible and they’re happy to answer any questions you may have. Your transition coordinator works Wednesdays and Thursdays at UCLH, and Fridays at Great Ormond Street.
Your transition coordinator provides the following services:
- Administrative support to you and your family
- Tours around UCLH before your first appointment
- Runs regular focus groups to improve and develop the transition service. These focus groups will give you the opportunity to meet other young people and interact in an informal environment. You will be able to share and discuss your own experiences. Please contact us for further information.
Your transition coordinator can also help with the following:
- Queries related to your transition
- Change of address, updating GP surgery details, telephone number, email address and next of kin details
- Confirm clinic appointments, date, time and location
- Put you in touch with a member of staff
- Provide you with information about our focus groups and when they will be taking place
- Post out an information pack.
