Data entered by patients

To keep your health record up to date, MyCare allows you to enter your own health information into your UCLH record. During the seven days before most appointments, you can use MyCare UCLH to review and update the list of your Medications, Allergies, and Health Issues. If made available to you, you will also be able to complete a questionnaire that relates to the reason for your appointment. You will be able to enter these updates only once every 30 days or longer.

Appointment Preparation

1. During the seven days before your appointment, you can log into MyCare and find a new tab under To Do. It’s called: Appointment Preparation.

2. When you select the Appointment Preparation option, you’ll see four icons: Medications, Allergies, Health Issues and Questionnaires.

1. If you have started taking a new medication, click on the Medication icon and select + REPORT A MEDICATION to add a new medication to your list.

2. Type the drug (medication) name in the search field and select the correct one from the list.

3. Add the date you are inserting the data and an optional comment. Click ACCEPT to move forward.

4. If this information is correct, click NEXT to confirm.

1. If you are allergic to something and you don’t have that information in your record yet, click the Allergies icon and then + REPORT AN ALLERGY.

2. Start typing the allergy name in the search field and select the correct one from the list.

3. Add the date you are inserting the data, an optional comment and click ACCEPT.

4. If this information is correct, click NEXT to submit your selection.

1. If you are experiencing a new health problem, it’s important you add it to your record so you can discuss it with your clinician. To do so, select the icon Health
Issues, and click + REPORT A HEALTH ISSUE.

2. Start typing the health issue name in the search field and select the correct one from the list.

3. Add the date you are inserting the data and an optional comment, then click ACCEPT.

4. If this information is correct, click NEXT to submit your selection.

1. At times, a questionnaire may be made available to you automatically due to the reason for your appointment.

2. Complete the questionnaire.

3. On the last page you can review your answers. If you notice a mistake you can click the pencil icon to go back to that section and edit it. Or click Back to go to the previous page. If the form is accurate, click Submit to send.
